Guest Intake Worksheet & House Rules

Below is the Paradise Recovery Housing Guest Intake Worksheet and our Sober Living House Rules & Guest Agreement. Please read it carefully. This worksheet is completed and signed with management at the time of admission — it is provided here so you know exactly what to expect before you move in.

Fee Clarification: Rent is $199.00 due weekly PLUS a $50.00 intake fee due at time of admission. For every urine screen sent to the lab, a $25.00 fee will be assessed to the client at that time. No refunds will be given if a client moves out or is asked to leave.

Sober Living House / Rules Guest Agreement

This Agreement is between Paradise Recovery Housing and the guest named at intake. As of the date listed, all Rules and Regulations are expected to be followed without exception. The final signature indicates that the guest fully understands this agreement and will abide by the terms herein.

A Sober Living House strives to provide a positive environment by offering a stable, clean, supportive, upbeat environment that functions as a safe “home base” from which guests can reintegrate into community, family, work, and life in general. The process of rebuilding a life can be difficult, but being able to take pride in one's living environment should be a positive component to one's recovery. Below are the terms required to remain in the Sober Living House.

Code of Conduct and House Rules — these must be followed at ALL times.

By signing the worksheet at intake, the guest signifies they have reviewed this Code of Conduct and all associated rules with management, have been given ample opportunity to ask questions, and understand and agree with the Code and rules herein. The worksheet is signed by both the guest and management.
